Types of aids/appliances
Persons with locomotor disability
- All prosthetic and orthotic devices, mobility aids, surgical foot wears, MCR chappals, all types of devices for ADL (activity of daily living) as recommended by expert committee from time to time.
- High end prosthesis - High end Prosthesis (Below Knee, Above Knee, Below elbow and Above elbow) to Persons with disabilities having 80% and above disability. Extent of subsidy would be Rs.30,000/-.

Chief Minister Relief Fund Scheme
The Chief Minister Relief Fund (CMRF) is a scheme that operates in Uttar Pradesh to relieve some of the financial pressures faced by people within the country. Segments of the populace in Uttar Pradesh that have been affected by situations of natural calamities that have affected people such as floods, droughts, and fire accidents, among others, will be assisted through this scheme to receive some financial benefits that will help them to overcome such situations, which can often happen all of a sudden and be challenging to pay for, citing the rising costs of healthcare in the state today
Eligibility Criteria for CMRF Scheme
Before availing of the benefits of the Chief Minister Relief Fund (CMRF) made available through donations for the CM Relief Fund providing P.A.N. number, one may have to check if they are eligible for this scheme or not based on the stated conditions. The people eligible for the Chief Minister Relief Fund (CMRF) will be those families that are B.P.L. or considered to be Below the Poverty Line, or a family that needs financial assistance for critical illness is only eligible to seek relief from CMRF. An application has to be submitted for the same. The applicant must submit certain documents involving an application with a BPL card, an Aadhar card, an Original e-income Certificate, and an RSBY Card. The rules of the scheme are shaped in a particular way to provide financial assistance to the poorest of the poor people, people who are regarded as eligible unemployed, patients who have been or are currently undergoing some treatment or preferably suffering from deadly diseases, qualified people with disabilities or on the accidental death of the head of the family or the earning member of the family The family that is considered Below The Poverty Line must also submit certain specific documents. These documents are an application with a BPL card and Aadhar card, Final Bills in original, and the person's Discharge summary. If the concerned patient wants to take the treatment, then instead of the final bills and discharge summary, they may submit an estimate from the hospital for the treatment. It shall be noted that the Medico-Legal Cases are not entertained for the sanction of relief under CMRF.
Grant Scheme for purchasing/repairing of Artificial Limbs and Hearing Aids for the Persons with Disabilities
Objective and Purpose
The main objective of this scheme is to provide financial grant to person with disabilities to buy artificial limbs and auxiliary equipment whose (employed or self employed) or whose family's income (in case of dependant) is not more than the decided BPL limit i.e., Rs.46080/- for rural areas and Rs.56460/- for urban areas from all sources and this annual income is scaled for per family or as per the amendments made by the Uttar Pradesh Government.
Rate of Grant
Under this scheme, for the purchase of artificial limbs and auxiliary equipment for person with disabilities, maximum amount of financial grant will be Rs.8000/- or the grant as decided by or amended by the Uttar Pradesh Government.
Eligibility and Conditions
- Any person with disability of any age who is the resident of Uttar Pradesh.
- Such person with disability, whose minimum disability is 40% and should be certified by Competent Authority of State Government. In the state of mental retardation, the state of the blocked or incomplete development of the mind which is specially characterized by abnormalities of growth, which has been certified by the competent authority.
- Medical Officer has recommended for the artificial limbs / accessories required for Person with Disabilities.
- Such person with disabilities who are not benefited by the Indian Government/State Government/Local Bodies in last three years for equipment. However, for the regular students of any educational institution this limit will be for one year.
Income
Persons with Disabilities who come under poverty line (Rs.46080/- in rural areas and Rs.56460/- in urban areas, this income has been decided per family annually) will be eligible for the grant or as per the guidelines of Uttar Pradesh government. To receive grant, income certificate issued by Hon'ble Member of Parliament, Hon'ble MLA, Mayor, Councilor, Nagar Panchayat Chairman, Zila Panchayat Chairman, First Class Magistrate of district, Tehsildar, BDO and Gram Pradhan will be eligible.
Details of Equipment
Under the scheme, as per the disability, artificial limbs/auxiliary equipment will be provided to the Persons with Disability:-
- Mobility Assistance Equipment like:- Tricycle, Wheelchair, C.P. Chair, Crutches, Walking Sticks and Walking s
- Students who are visually impaired will be provided with educational equipment like mathematical, abacus, geometry kits and Braille educational kits.
- Blind stick for visually impaired persons.
- For hearing impaired persons, different types of hearing-aided equipment and education kit.
- M.S.I.D. kit (Multi-Sensory Education Development Kit) for children and students with mental disabilities.
- Daily activity kits (A.D.L. Kit) to people free from leprosy.
- In case of multi-disability or person with disabilities who need more than one assistance/aid equipment, they will be provided with maximum Rs.10000/- financial grant in one go.
